We want reporting to be as simple as possible. Select the category below that best matches your concern. This list is not exhaustive but is designed to help you quickly find the right resource.
Submit comments, suggestions, or concerns about any aspect of the university experience that don't fall under specific reporting categories.
Report accidents or unsafe conditions that could have resulted in harm, physical hazards, unsafe facilities, equpment malfuntions, or environmental health concerns that pose safety risks to the campus community.
Submit concerns about a student whose behavior suggests they may need support or intervention. This includes situations in which the student may present a threat to themselves or others. The Behavioral Intervention Team will assess and provide appropriate assistance.
Report crimes and public safety concerns on campus as required by federal Clery Act regulations.

Report websites, documents, or digital content that are not accessible to individuals with disabilities. Reports help ensure compliance with accessibility standards.
Report employment-related concerns, such as workplace discrimination or harassment (excluding sex-based issues covered by Title IX).

Report student misconduct concerns, including hazing activities. Hazing includes any action that humiliates, degrades, or risks harm as part of joining or participating in a group.
Confidentially report suspected fraud, waste, abuse, legal violations, or unethical conduct within the university. Reporters are protected from retaliation under whistleblower policy.
