Vaccination confirmation and exemptions

In accordance with Washington and Oregon state requirements, WWU students, faculty, and staff must either confirm they are fully vaccinated against COVID-19* or receive from WWU a medical or religious exemption from vaccination to access campus facilities. Those who choose not to be fully vaccinated must also submit proof of a negative COVID-19 test as instructed. *According to the Centers for Disease Control and Prevention (CDC), you are fully vaccinated for COVID-19 two weeks or more after receiving the second dose in a two-dose series (Pfizer-BioNTech or Moderna), or two weeks or more after receiving a single dose of the Johnson & Johnson (J&J/Janssen) vaccine. WWU will also recognize international COVID-19 vaccines that have been approved by the World Health Organization (WHO).

Vaccination confirmation and requests for exemptions submitted to WWU will be kept confidential and shared only with essential personnel if needed to inform the university’s COVID-19 response.

For all three categories below, online forms will be submitted through myWWU. Downloaded forms should be submitted via email to covid.compliance@wallawalla.edu.

Vaccination confirmation
Complete an online form to provide information about the type of vaccine you received, and when and where you received it. Parents of students under age 18 should also complete and return via email the COVID-19 Vaccination Declaration for Parents/Guardians of Minor Students Confirming Vaccination form.

Students may complete an attestation form to report vaccination, but faculty and staff are required by Washington state to also submit their official documentation record.

Medical exemption
Complete an online form, and then you and your health care provider will also complete and return via email the COVID-19 Vaccination Declaration Request for Medical Exemption form. Parents of students under age 18 should also complete and return via email the COVID-19 Vaccination Declaration for Parents/Guardians of Minor Students Requesting Medical Exemption form.

Religious exemption
Complete an online form sharing the religious principles that form a foundation for your objection to the COVID-19 vaccine. Parents of students under age 18 should also complete and return via email the COVID-19 Vaccination Declaration for Parents/Guardians of Minor Students Requesting Religious Exemption form.
