Retirement Information

Salary Reduction Agreement & Beneficiary Designation Form is used to designate a voluntary contribution to the Adventist Retirement Plan, and to name a Primary and Secondary Beneficiary.

Alternative Beneficiary Designation Form  is to be used when someone other than a spouse is named as beneficiary of an Employee's Adventist Retirement Plan account. The spouse must sign this form.
