For LASIK, Acupuncture, Chiropractic, Adult Immunization, Dental, Vision, Massage use Claims Reimbursement Request Form

For out of the network expenses use BlueCard PPO Out-of-Network

For prescriptions use Medco RX Reimbursement Form

For Dental use United Concordia Dental Claim Form

For all other Medical Claims use Reimbursement Form

• Information you will need for filling out this form:
  • Identification Number= Your SDA# shown on the front of your Blue PPO card
  • Group Number= Leave Blank
